INDICATIONS

  1. Symptomatic relief of mild to moderate pain of acute musculoskeletal disorders.
  2. The orphenadrine component is indicated as an adjunct to rest, physical therapy, and other measures for the relief of discomfort associated with acute painful musculoskeletal conditions.

The mode of action of orphenadrine has not been clearly identified, but may be related to its analgesic properties. Norgesic (orphenadrine citrate with aspirin and caffeine) and Norgesic (orphenadrine citrate with aspirin and caffeine) Forte do not directly relax tense skeletal muscles in man.

DOSAGE AND ADMINISTRATION

Norgesic (orphenadrine citrate with aspirin and caffeine) : Adults 1 to 2 tablets 3 to 4 times daily.

Norgesic (orphenadrine citrate with aspirin and caffeine) Forte: Adults ½ to 1 tablet 3 to 4 times daily.

HOW SUPPLIED

Norgesic (orphenadrine citrate with aspirin and caffeine) tablets can be identified by their two layers colored white and yellow. Each round tablet is embossed “NORGESIC (orphenadrine citrate with aspirin and caffeine) ” on one side and “3M” on the other and contains orphenadrine citrate (2-dimethylaminoethyl 2-methylbenzhydryl ether citrate) 25 mg, aspirin 385 mg, and caffeine 30 mg.

Norgesic (orphenadrine citrate with aspirin and caffeine) Forte tablets are exactly twice the strength of Norgesic (orphenadrine citrate with aspirin and caffeine) . They are identified by their scored capsule shape and by their two layers colored white and yellow. Each capsule shaped tablet is embossed “NORGESIC (orphenadrine citrate with aspirin and caffeine) FORTE” on one side and “3M” on the other and contains orphenadrine citrate 50 mg, aspirin 770 mg, and caffeine 60 mg.

Norgesic (orphenadrine citrate with aspirin and caffeine) and Norgesic (orphenadrine citrate with aspirin and caffeine) Forte also contain: lactose, polyethylene glycol, povidone, starch, sucrose, zinc stearate, and D&C yellow #10.

Norgesic (orphenadrine citrate with aspirin and caffeine) : Bottles of 100 tablets (NDC 0089-0231-10) and 500 tablets (NDC 0089-0231-50).

SIDE EFFECTS

Side effects of Norgesic (orphenadrine citrate with aspirin and caffeine) or Norgesic (orphenadrine citrate with aspirin and caffeine) Forte are those seen with aspirin and caffeine or those usually associated with mild anticholinergic agents. These may include tachycardia, palpitation, urinary hesitancy or retention, dry mouth, blurred vision, dilatation of the pupil, increased intraocular tension, weakness, nausea, vomiting, headache, dizziness, constipation, drowsiness, and rarely, urticaria and other dermatoses. Infrequently, an elderly patient may experience some degree of confusion. Mild central excitation and occasional hallucinations may be observed. These mild side effects can usually be eliminated by reduction in dosage. One case of aplastic anemia associated with the use of Norgesic (orphenadrine citrate with aspirin and caffeine) has been reported. No causal relationship has been established. Rare G.I. hemorrhage due to aspirin content may be associated with the administration of Norgesic (orphenadrine citrate with aspirin and caffeine) or Norgesic (orphenadrine citrate with aspirin and caffeine) Forte. Some patients may experience transient episodes of light-headedness, dizziness or syncope.

WARNINGS

Reye's Syndrome may develop in individuals who have chicken pox, influenza, or flu symptoms. Some studies suggest a possible association between the development of Reye's Syndrome and the use of medicines containing salicylate or aspirin. Norgesic (orphenadrine citrate with aspirin and caffeine) and Norgesic (orphenadrine citrate with aspirin and caffeine) Forte contain aspirin and therefore are not recommended for use in patients with chicken pox, influenza, or flu symptoms. Norgesic (orphenadrine citrate with aspirin and caffeine) and Norgesic (orphenadrine citrate with aspirin and caffeine) Forte may impair the ability of the patient to engage in potentially hazardous activities such as operating machinery or driving a motor vehicle; ambulatory patients should therefore be cautioned accordingly.

Aspirin should be used with extreme caution in the presence of peptic ulcers and coagulation abnormalities.

Usage In Pregnancy

Since safety of the use of this preparation in pregnancy, during lactation, or in the childbearing age has not been established, use of the drug in such patients requires that the potential benefits of the drug be weighed against its possible hazard to the mother and child.

Pediatric Use

Safety and effectiveness in pediatric patients have not been established.

PRECAUTIONS

Confusion, anxiety and tremors have been reported in a few patients receiving propoxyphene and orphenadrine concomitantly. As these symptoms may be simply due to an additive effect, reduction of dosage and/or discontinuation of one or both agents is recommended in such cases.

Safety of continuous long term therapy with Norgesic (orphenadrine citrate with aspirin and caffeine) and Norgesic (orphenadrine citrate with aspirin and caffeine) Forte has not been established; therefore, if Norgesic (orphenadrine citrate with aspirin and caffeine) or Norgesic (orphenadrine citrate with aspirin and caffeine) Forte is prescribed for prolonged use, periodic monitoring of blood, urine and liver function values is recommended.

CONTRAINDICATIONS

Because of the mild anticholinergic effect of orphenadrine, Norgesic (orphenadrine citrate with aspirin and caffeine) or Norgesic (orphenadrine citrate with aspirin and caffeine) Forte should not be used in patients with glaucoma, pyloric or duodenal obstruction, achalasia, prostatic hypertrophy or obstructions at the bladder neck. Norgesic (orphenadrine citrate with aspirin and caffeine) or Norgesic (orphenadrine citrate with aspirin and caffeine) Forte is also contraindicated in patients with myasthenia gravis and in patients known to be sensitive to aspirin or caffeine.

The drug is contraindicated in patients who have demonstrated a previous hypersensitivity to the drug.

CLINICAL PHARMACOLOGY

Actions

Orphenadrine citrate is a centrally acting (brain stem) compound which in animals selectively blocks facilitatory functions of the reticular formation. Orphenadrine does not produce myoneural block, nor does it affect crossed extensor reflexes. Orphenadrine prevents nicotine-induced convulsions but not those produced by strychnine.

Chronic administration of Norgesic (orphenadrine citrate with aspirin and caffeine) to dogs and rats has revealed no drug-related toxicity. No blood or urine changes were observed, nor were there any macroscopic or microscopic pathological changes detected. Extensive experience with combinations containing aspirin and caffeine has established them as safe agents. The addition of orphenadrine citrate does not alter the toxicity of aspirin and caffeine.

The mode of therapeutic action of orphenadrine has not been clearly identified, but may be related to its analgesic properties.

Orphenadrine citrate also possesses anticholinergic actions.

COMMON BRAND NAME(S): Norgesic

USES: This medication is a combination of orphenadrine, aspirin, and caffeine. It is used to treat muscle spasms/pain. It is usually used along with rest, physical therapy, and other treatment.

Orphenadrine relieves pain and muscle spasms. Aspirin relieves pain and decreases swelling. Caffeine may help increase the effects of aspirin on pain.

HOW TO USE: Take this medication by mouth as directed by your doctor, usually 3 or 4 times a day. If you have stomach upset while taking this medication, take it with food or milk. Take this medication with a full glass of water (8 ounces/240 milliliters) unless your doctor directs you otherwise. Do not lie down for 10 minutes after taking this medication.

The dosage is based on your medical condition and response to treatment. Do not increase your dose or use this drug more often or for longer than prescribed. Your condition will not improve any faster, and your risk of side effects will increase.

This medication contains caffeine. Avoid drinking large amounts of beverages containing caffeine (coffee, tea, colas), eating large amounts of chocolate, or taking nonprescription products that contain caffeine.

Tell your doctor if your condition does not improve or if it worsens.

SIDE EFFECTS: Dry mouth, dizziness, drowsiness, lightheadedness, blurred vision, upset stomach, heartburn, nausea, vomiting, constipation, trouble sleeping, and increased urination may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.

To relieve dry mouth, suck (sugarless) hard candy or ice chips, chew (sugarless) gum, drink water, or use a saliva substitute.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Tell your doctor right away if you have any serious side effects, including: mental/mood changes (such as confusion, anxiety, hallucinations, nervousness, agitation), shaking (tremor), fast/pounding/irregular heartbeat, hearing changes (such as ringing in the ears, difficulty hearing), difficulty urinating, easy bleeding/bruising, fainting, eye pain, unusual tiredness, change in the amount of urine, yellowing eyes/skin, dark urine.

This drug may infrequently cause serious (rarely fatal) bleeding from the stomach or intestines. If you notice any of the following unlikely but serious side effects, stop taking this medication and consult your doctor or pharmacist immediately: black stools, vomit that looks like coffee grounds, persistent stomach/abdominal pain.

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, breathing trouble.

This drug may make you dizzy or drowsy or cause blurred vision. Do not drive, use machinery, or do any activity that requires alertness or clear vision until you are sure you can perform such activities safely. Limit alcoholic beverages.

Before having surgery, tell your doctor or dentist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).

This medicine may cause stomach bleeding. Daily use of alcohol and tobacco, especially when combined with this medicine, may increase your risk for stomach bleeding. Limit alcohol and stop smoking. Consult your doctor or pharmacist for more information.

This drug contains aspirin. Children and teenagers should not take aspirin if they have chickenpox, flu, or any undiagnosed illness or if they have recently received a vaccine. In these cases, taking aspirin increases the risk of Reye's syndrome, a rare but serious illness.

Older adults may be more sensitive to the side effects of this drug, especially confusion and stomach ulcer/bleeding.

Aspirin is not recommended for use during pregnancy. Consult your doctor before using this medication if you are or think you may be pregnant. Tell your doctor immediately if you become pregnant or think you may be pregnant while taking this medication. Do not use this medication during the last 3 months of pregnancy because of possible harm to the unborn baby or problems during delivery.

This medication passes into breast milk and may harm a nursing infant. Therefore, breast-feeding while using this drug is not recommended. Consult your doctor before breast-feeding.

Tell your doctor or pharmacist if you are taking other products that cause drowsiness including alcohol, antihistamines (such as cetirizine, diphenhydramine), drugs for sleep or anxiety (such as alprazolam, diazepam, zolpidem), muscle relaxants, and narcotic pain relievers (such as codeine).

Check the labels on all your medicines (such as allergy or cough-and-cold products) because they may contain ingredients that cause drowsiness. Ask your pharmacist about using those products safely.

This medication may interfere with certain laboratory tests (including dipyridamole-thallium imaging tests, certain urine tests), possibly causing false test results. Make sure laboratory personnel and all your doctors know you use this drug.

OVERDOSE: If overdose is suspected, contact a poison control center or emergency room immediately. US residents can call the US National Poison Hotline at 1-800-222-1222. Canada residents can call a provincial poison control center. Symptoms of overdose may include: unusual excitement, fast/irregular heartbeat, seizures, rapid breathing, mental/mood changes (such as confusion), change in the amount of urine.

What is aspirin/caffeine/orphenadrine (Norgesic)?

Aspirin is in a class of drugs called salicylates. Aspirin works reducing substances in the body that cause pain and inflammation. It is used to reduce pain, inflammation, and fever.

Caffeine is used in this combination to increase the pain relieving effects of aspirin and orphenadrine.

Orphenadrine is a muscle relaxant. It works by blocking nerve impulses (or pain sensations) that are sent to the brain.

Aspirin/caffeine/orphenadrine is used, along with rest and physical therapy, to treat injuries and other painful muscular conditions.

Aspirin/caffeine/orphenadrine may also be used for purposes other than those listed in this medication guide.

What is the most important information I should know about aspirin/caffeine/orphenadrine (Norgesic)?

Use caution when driving, operating machinery, or performing other hazardous activities. Aspirin/caffeine/orphenadrine may cause dizziness or drowsiness. If you experience dizziness or drowsiness, avoid these activities.

Use alcohol cautiously. Alcohol may increase drowsiness and dizziness while taking aspirin/caffeine/orphenadrine. Also, in combination with aspirin, alcohol can be damaging to the stomach.

Call your doctor immediately if you experience a rash, itching, a fever, or nasal congestion during treatment with aspirin/caffeine/orphenadrine.

Watch for bloody, black, or tarry stools or bloody vomit. This could indicate damage to the stomach.

Do not take more of this medication than is prescribed. If the pain is not being adequately treated, talk to your doctor.

Who should not take aspirin/caffeine/orphenadrine (Norgesic)?

Before taking this medication, tell your doctor if you have

  • kidney disease,
  • liver disease,
  • a stomach or intestinal ulcer or obstruction,
  • a bleeding or blood clotting disorder,
  • glaucoma,
  • an enlarged prostate or a blockage of the urinary tract (difficulty urinating), or
  • myasthenia gravis.

You may not be able to take aspirin/caffeine/orphenadrine, or you may require a dosage adjustment or special monitoring during treatment if you have any of the conditions listed above.

It is not known whether aspirin/caffeine/orphenadrine will be harmful to an unborn baby. Do not take aspirin/caffeine/orphenadrine without first talking to your doctor if you are pregnant or could become pregnant during treatment.

It is not known whether aspirin/caffeine/orphenadrine passes into breast milk. Do not take aspirin/caffeine/orphenadrine without first talking to your doctor if you are breast-feeding a baby.

Aspirin/caffeine/orphenadrine is not approved for use by children younger than 12 years of age. Do not use any medication that contains aspirin to treat a child or teenager who has a fever, flu symptoms, or chicken pox without first talking to a doctor. In children younger than 20 years of age, aspirin may increase the risk of Reye's syndrome, a rare but often fatal condition.

How should I take aspirin/caffeine/orphenadrine (Norgesic)?

Take aspirin/caffeine/orphenadrine exactly as directed by your doctor. If you do not understand these directions, ask your pharmacist, nurse, or doctor to explain them to you.

Take each dose with a full glass of water.

Do not take more of this medication than is prescribed. If the pain is not being adequately treated, talk to your doctor.

Drowsiness or dizziness caused by orphenadrine may be increased by other drugs such as antidepressants, alcohol, antihistamines, sedatives (used to treat insomnia), other pain relievers, anxiety medicines, and muscle relaxants. Together, these medicines may cause dangerous sedation, possibly resulting in unconsciousness and death. Tell your doctor about all medicines that you are taking, and do not take any medicine without first talking to your doctor.

Do not take aspirin/caffeine/orphenadrine without first talking to your doctor if you are taking any of the following medicines:

  • an anticoagulant such as warfarin (Coumadin), heparin, enoxaparin (Lovenox), dalteparin (Fragmin), danaparoid (Orgaran), ardeparin (Normiflo), or tinzaparin (Innohep);
  • a nonsteroidal anti-inflammatory drug (NSAID) such as ibuprofen (Motrin, Advil, Nuprin, others), ketoprofen (Orudis, Orudis KT, Oruvail), naproxen (Naprosyn, Anaprox, Aleve), diclofenac (Voltaren, Cataflam), nabumetone (Relafen), oxaprozin (Daypro), piroxicam (Feldene), etodolac (Lodine), fenoprofen (Nalfon), flurbiprofen (Ansaid), indomethacin (Indocin), ketorolac (Toradol), sulindac (Clinoril), or tolmetin (Tolectin); or
  • another salicylate such as aspirin (Acuprin, Ecotrin, Ascriptin, Bayer, others); choline salicylate and/or magnesium salicylate (Magan, Doan's, Bayer Select Backache Pain Formula, Mobidin, Arthropan, Trilisate, Tricosal), or salsalate (Disalcid).

You may not be able to take aspirin/caffeine/orphenadrine, or you may require a dosage adjustment or special monitoring if you are taking any of the medicines listed above.

Drugs other than those listed here may also interact with aspirin/caffeine/orphenadrine. Talk to your doctor and pharmacist before taking any prescription or over-the-counter medicines.
